Simvastatin, an inactive lactone, is synthesized by Aspergillus Oryzae from its fermentation products. Its active structure catalyzes the inhibition of 3-hydroxy-3-methylglutaryl-CoA (HMG CoA) reductase.
Simvastatin first appeared in the UK back in 1988. Clinical studies have shown that simvastatin reduces total cholesterol levels, as well as low-density cholesterol and very-low-density cholesterol.
Simvastatin is also able to increase high-density cholesterol lipoprotein and lower triglycerides. Simvastatin is suitable for hypercholesterolemia primary and low-density Lipoprotein Cholesterol. It can also increase high-density Lipoprotein Cholesterol and lower serum triglyceride.
Simvastatin is an inhibitor of HMG CoA reducetase and can catalyze b-methyl-b-hydroxyglutarate CoA into b-methyl-p-a-dihydroxyvaleric acids.
Simvastatin, after oral administration, is hydrolyzed to the corresponding b-hydroxy acid. This is one of the most effective and safe drugs for lowering lipids. It can lower the serum levels of total cholesterol, low density lipoproteins, very low density lipoproteins, triglycerides and increase high-density cholesterol lipoproteins.

1.Simvastatin should not be used with anticoagulants. The dosage of anticoagulant must be reduced. Simvastatin shouldn’t be taken with immunosuppressive medications, phenoxyaromatic acids, niacin or erythromycin. Otherwise, it can cause myopathy or even rhabdomyolysis.
2. Combining simvastatin with resin-based lipid-lowering medications can enhance lipid-lowering effect and reduce dosage as well as side effects. It can be used in conjunction with probucol to prevent atherosclerosis, coronary heart disease and other cardiovascular diseases.
Simvastatin increases the effectiveness of coumarin-anticoagulants, such as Warfarin. The dosage of warfarin should be reduced if used together.
3. Simvastatin may moderately increase the anticoagulant effects of coumarins. When simvastatin combined with other drugs which significantly inhibit cytochrome P450A4 when taken at therapeutic doses there is an increased risk of Rhabdomyolysis. Caution should be used.
Simvastatin can increase the risk of rhabdomyolysis when combined with drugs that inhibit cytochrome P4503A4 at therapeutic dosages. This includes fiber acid derivatives and niacin.
5. Combining simvastatin with beta drugs can increase the severity and incidence of myopathy.
6. Simvastatin is a mild anticoagulant that can improve the effect of anticoagulants.
